The U.S. National Transportation Safety Board (NTSB) has concluded that the helicopter crash which claimed the lives of former Access Holdings CEO Herbert Wigwe, his wife, and son, was caused by the pilot’s spatial disorientation during the flight.

In its final report, the NTSB also blamed the helicopter company for failing to properly manage safety procedures. Investigators said the company did not ensure that pilots completed flight risk analyses, reported maintenance issues, or followed required regulations before departure. The helicopter was flying under visual flight rules but encountered poor weather that required instrument-based navigation, which the pilot was not adequately prepared for.
The report highlights serious lapses in the company’s safety oversight, contributing to the tragic February 2024 crash.
